This is a remote position within WI, ideally with a candidate living near the Milwaukee area. This role is a key member of our interdisciplinary team, where you will be responsible for partnering with your fellow colleagues to provide top quality coordination of care to our members. They are looking to you in providing assistance when it comes to living their daily lives. The RN is responsible for facilitating the coordination, communication, negotiation, and collaboration of streamlined healthcare delivery processes on behalf of the member. This is a rewarding role where you will be changing the lives of the members you serve!

  • Assist with medical transitions of care- completes engagement with the member while inpatient, discharge coordination, and follow-up after discharge 
  • Employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues.
  • Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.
  • Contact and coordinate with acute and primary care providers.
  • Identify and coordinate clinical and functional supports needed for the member’s health and safety.
  • Educate on and promote prevention and wellness and mitigate risk when assessed.
  • Create member care plans, updating frequentlyand develop smart goals.
  • Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas.
  • Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed.
  • Identifies and resolves barriers that hinder effective care.
  • Completes health risk screenings for members

Required Qualifications

  • Bachelor's degree
  • Minimum of 2 years of clinical acute care experience
  • Licensed Registered Nurse (RN) in WI with no disciplinary action.

Preferred Qualifications

  • Experience with case management, discharge planning and patient education for adult acute care
  • Managed care experience

  • Workstyle: Hybrid Home
  • Work Location: Remote within Wisconsin
  • Travel: Minimal travel required
  • Typical Work Days/Hours: Monday through Friday, 8:30 am – 5:00 pm CST.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$71,100 - $97,800 per year


 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
