Community Resource Outreach Representative - Bilingual Spanish/English

Humana Healthy Horizons in Florida is seeking a Bilingual Spanish/English Community Resource Outreach Representative who represents the company by outreaching to members to solve for social determinant of health needs. The Outbound Contacts Representative performs basic administrative, clerical, and documentation duties and customer support.
The Outbound Contacts Representative:
- Performs necessary outreach to members and follows up as needed to address social determinants of health needs.
- Ensures responses to member inquiries are completed on a timely basis to achieve service and quality standards.
- Collaborates with outside organizations to ensure services are available to meet member needs.
- Documents member contacts and strives to close the loop on all referrals.
- Decisions are limited to defined parameters around work expectations, quality standards, priorities and timing, and works under close supervision and/or within established policies/practices and guidelines with minimal opportunity for deviation.

Required Qualifications
- Minimum of one (1) year of customer service experience.
- Bilingual English/Spanish. Must be able to speak, read and write in both languages without limitations or assistance. See Additional Information on testing.
- Proficiency in Microsoft Office applications, including Outlook, Word and Excel.
Preferred Qualifications
- Associate's or Bachelor's Degree.
- Experience in healthcare.
- Call center experience.
- Experience in CGX.
Additional Information
- Workstyle: Home. Home workstyle is defined as remote but will use Humana office space on an as needed basis for collaboration and other face-to-face needs.
- Location: must reside in Florida.
- Travel: may require travel once/year for in person meeting.
- Typical Work Days/Hours: Monday – Friday, 8:00 – 4:30 pm EST.
Language Proficiency Testing
Any Humana associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.
WAH Internet Statement
To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria:
