Clinical Pharmacist Specialist - Evernorth - Remote (Chronic Conditions) - Premium Solutions Multidisciplinary Team

GENERAL PURPOSE
- To provide Clinical Pharmacist support to the Premium Solutions and Everyday Health coaching and case management programs relating to medication information and clinical consults
- Improve health outcomes through telephonic consults directly with customers regarding clinical education to improve medication adherence and understanding of medications
RESPONSIBILITIES
Provides clinical consultation and drug information to the Premium Solutions and Everyday Health Engagement coaches/CMs regarding:
* general medication questions
* medication regimen reviews/consultations
* medication reviews for polypharmacy, safety concerns, adherence, gaps in care, adherence to evidence based treatment guidelines, etc
* drug information (side effects, interactions, etc)
Provides telephonic consultation with customers engaged in health coaching or case management to improve adherence and understanding of their medications
Participates in multidisciplinary team rounds
Identifies educational opportunities as related to medication; provides presentations and in-services to coaches/CMs as needed
Promotes integration and teamwork within the coaching and case management teams.
Actively participates in team meetings and case discussions
Completes project work as assigned

MINIMUM REQUIREMENTS
- Doctor of Pharmacy degree required
- Completion of ASHP accredited residency
- A minimum of 3-5 years of strong clinical experience (Clinical Specialist, etc)
- Board Certification strongly preferred.
- Current active pharmacist license in the state of residence required
- Previous customer facing experience and health plan experience preferred
REQUIRED SKILLS
- Strong communication skills (written and verbal)
- Strong clinical background and knowledge of national treatment guidelines and evidence based standards of care for common chronic conditions
- Expertise in drug information retrieval and medication regimen assessment
- Ability to work within a multi-disciplinary team
- Experience with managing a variety of tasks and strong prioritization skills
- Demonstrated ability to educate patients, colleagues and staff members
- Successful ability to assess complex issues, to determine and recommend solutions and to resolve problems
- Demonstration of strong effective teamwork, negotiation, conflict management, decision-making and problem solving skills
- Ability to work within a changing business environment and balance patient advocacy with business needs
If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.For this position, we anticipate offering an annual salary of 111,900 - 186,500 USD / yearly, depending on relevant factors, including experience and geographic location.
This role is also anticipated to be eligible to participate in an annual bonus plan.
