Responsibilities
The Behavioral Health Clinician (BHC) position is a behavioral health provider who works in a consultative role, providing behavioral health interventions to primary care patients within the LMG clinics.
The BHC is an integral member of the primary care team whose role is to help address a broad spectrum of behavioral health needs in the primary care patient population. Their goal is to aid in early identification and provide problem focused interventions that promote wellness and long-term prevention for as many patients as possible. The BHC provides consultative services for behavioral health issues related to physical health conditions in the form of assessment, management and triage of psychological and social problems in the clinic patient population.
This position requires advanced level skill in delivering evidence-based interventions targeted to improve overall health for patients with a variety of emotional and behavioral problems. The work includes the BHC meeting with patients individually for an assessment and the providing recommendations to the primary care team to enhance the patient's care plan. When indicated, the BHC may also work with patients for several subsequent sessions to provide on-going skills training and coaching to improve health and functioning. The BHC will also support a smooth coordination of care with community partners for patients who require services not provided in the LMG clinic.
Qualifications
Education:
Master of Social Work degree from an accredited school of social work,
Experience:
Minimum of two years of clinical experience or healthcare related field.
Skills:
Screen for common mental health problems. Able to conduct Cognitive-Behavioral or solution focused interventions. Recognizes and advocates for patients regarding behavioral health strategies/interventions to the referring PCP. Promotes health behavior change through evaluation, brief intervention and education. Works with multidisciplinary team to help support the patient’s health. Coordinates with LMG providers and community partners to refer for appropriate mental health therapy.
Demonstrated competencies in assessment of emotional, cognitive and behavioral disorders in child, adolescent, adult and/or geriatric patients (depending upon population served by specific position). Demonstrated competency in assessment cognitive and psychological problems associated with, and/or exacerbating medical conditions. Experience using motivational interviewing, skills in conducting group and individual psycho/social education classes desirable.
