About the Role

An exciting position is available to lead the Transition Care Program (TCP) and Home-based Allied Health Service at St Vincent’s Hospital Melbourne.

As the Manager of these two Commonwealth funded services you will provide efficient, contemporary operational management and effective coordination of the services, including human and financial aspects and brokered services.

To be successful in this role you will be an experienced leader with deep knowledge of geriatrics and demonstrated aptitude in healthcare service provision. Dynamic and innovative in your approach, you thrive managing daily clinical complexities; balanced with efficient and effective execution and implementation of strategic initiatives. You are a good communicator, with an empathetic and supportive approach and easily develop meaningful relationships with all those you work with.

​Day to Day Responsibilities

  • Support effective operational and strategic leadership across the TCP and HBAH Program

  • Support the development of clinical policies, plans and programs

  • Lead, co-ordinate and manage staff within the TCP and HBAH Program

  • Manage staff and client safety, including promoting a culture of safety

  • Undertake service planning, organising, integration and coordination is aligned to the mission and organisational priorities

  • Lead quality and continuous improvement initiatives, and service development activities

  • Establish sound, collaborative working relationships with key stakeholders

The classification for this position will range from:

Allied Health Grade 4 -

Nurse Unit Manager Level 1 NM10 - $127,722.40 p/a (pro-rata for part-time)

Nurse Unit Manager Level 2 NM11 - $132,371.20 p/a (pro-rata for part-time)

What You'll Bring

  • Demonstrated experience in leading and facilitating sustained continuous improvement of interdisciplinary health services

  • Demonstrated operational management skills, including budget management and procurement of contracted services.

  • Demonstrated ability to build and maintain effective relationships with key stakeholders

  • Post-graduate level qualification in management or commencement thereof (desirable)

About Transition Care Program and Home-based Allied Health Team

The Transition Care Program consists of a multidisciplinary team whom are resourced to follow the client through the service system and avoid unnecessary extension of hospital admissions and/or premature admission to residential care.

The intent of the Transition Care Program is to provide short-term support and active management for older people at the interface of the acute/subacute and residential aged care sectors. The Transition Care Program enables older people to have more time in a non-hospital environment to complete their restorative process and finalise access to their long-term care arrangements.

The Home-based Allied Health team is a Commonwealth Home Support Program (CHSP) which focuses on restoring, or maintaining people’s independence, improving functioning and wellbeing to support them to

remain living at home. The team includes: Physiotherapy, Occupational Therapy, Speech Pathology, Podiatry, Dietetics and an Allied Health Assistant.
