We're currently looking for independently licensed mental health professionals to join the Care Navigation team (LCSW, LMFT, LPC required) to support members seeking support with substance use. The SUD Clinical Care Navigator will provide initial assessment, support, referral, and ongoing care coordination for members within Spring Health’s innovative SUD program. 

The shift for this position will be Monday-Friday, 9-5pm (respective time zone).

What you’ll be doing:

• Telephonic screening, brief intervention, and referral of members to a treatment provider (SBIRT model).
• Apply active listening, psycho-education, and motivational interviewing skills to understand each member’s needs and provide support as they set goals to decrease substance use and/or to achieve successful recovery.
• Using ASAM criteria, identify an appropriate level of care, facilitate referral, and coordinate care with a Spring Health SUD program or provider.
