Position SummaryPosition provides clinical social work services to referred WashU patients to complete psychosocial assessments; plans and implements needed social work services; identifies barriers to care and find solutions to the barriers; connects patients with community resources; utilizes resources available; and collaborates with healthcare team and outside agencies to provide patient-centered care.

Job Description

Primary Duties & Responsibilities:

• Provides direct clinical social work services to referred WU patients telephonically.
• Completes psychosocial assessments for referred WU outpatient patients.
• Plans and implements social work services to assigned patients.
• Completes screenings and assessment tools to meet the needs of the patient.
• Identifies barriers to accessing care and finding solutions for those barriers.
• Connects patients with community resources to alleviate barriers to care and improve health outcomes.
• Utilizes resources available to patients to manage their continuum of care.
• Collaborates with the health care team and human service agencies to meet the needs of patients and families.

Master's degree - Social Work

Certifications:

Basic Life Support - American Heart Association, Basic Life Support - American Red Cross, Basic Life Support - CPR St Louis, Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation, Licensed Clinical Social Worker (LCSW) - Missouri Division of Professional Registration, Licensed Master Social Worker (LMSW) - Missouri Division of Professional Registration

Required Qualifications

• Master’s degree in Social Work.
• Currently a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the state of Missouri and/or Illinois depending on location.
• Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).
