Social Worker MSW (Remote) - Faculty Practice Plan

Primary Duties & Responsibilities:
- Provides direct clinical social work services to referred WU patients telephonically.
- Completes psychosocial assessments for referred WU outpatient patients.
- Plans and implements social work services to assigned patients.
- Completes screenings and assessment tools to meet the needs of the patient.
- Identifies barriers to accessing care and finding solutions for those barriers.
- Connects patients with community resources to alleviate barriers to care and improve health outcomes.
- Utilizes resources available to patients to manage their continuum of care.
- Collaborates with the health care team and human service agencies to meet the needs of patients and families.

Required Qualifications
- Master’s degree in Social Work.
- Currently a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the state of Missouri and/or Illinois depending on location.
- Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).
