About the Role:
The Senior Care Navigator is the ultimate guide to Octave for our prospective clients. This role will contribute to Octave’s mission of expanding access by introducing clients to our innovative practice, and guiding them through the behavioral health landscape. They will be involved in interacting with our highest-needs clients, ensuring they receive the support in their time of need. Senior Care Navigators are often involved in inter-departmental initiatives that help aid and inform the growth of the company.
Responsibilities Include:
- Conduct outreach to understand higher risk clients’ needs, and take action when necessary with the support of clinical staff.
- Support clients along their care journey, from entry into our system, all the way to their first appointment with their provider and beyond
- Demonstrates a keen operational eye in order to execute on frequently iterated, complex workflows.
- Leverage clinical expertise and experience to determine the most suitable provider for our clients based on needs, preferences and availability.
- Assist in collaborative projects across departments to benefit the organization.
- Collaborate with providers to offer initial clinical insights as applicable and support them with client care transitions and caseload utilization.
- Gain proficiency in our tools and workflows, especially when they evolve as we grow and scale, to successfully navigate our internal operations.
- Embody Octave’s mission to deliver extraordinary care in every interaction with clients and colleagues.
Required Qualifications:
- Master’s level mental health/clinical degree.
- Preferred 2-3 years experience working in a Care Navigation/Coordination/Intake capacity, specifically within the mental health or healthcare arena.
- Experience with telephonic crisis intervention.
- Excellent verbal and written communication and can adapt your approach to multiple groups.
- Detail oriented and impeccable organizational skills are a must.
- Operational experience within a fast-growth healthtech organization is ideal.
- Self-starter, willing to roll up your sleeves and be proactive and solution oriented.
