Position Summary

The Nurse Care Manager (NCM) is responsible for providing enhanced care coordination to patients attributed to WUSM value-based care agreements. Primary responsibilities include providing telephonic transitional care outreach to patients recently discharged from an inpatient stay, seen in the Emergency Department (ED) for potentially avoidable treatments, or those with multiple complex comorbidities. The NCM will complete transitions of care assessments, medication reconciliation, provide disease specific education, and assist to schedule follow-up appointments to reduce likelihood of readmission and/or avoidable ED use.

Job Description

Primary Duties and Responsibilities

  • Integrates evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency, and the delivery of healthcare for medical home patients.
  • Assesses, plans, implements, coordinates, monitors, documents, and evaluates options and services to optimize the member’s health status.
  • Develops systems of care that monitor member’s progress and promotes early intervention in acute care situations.
  • Establishes and promotes a collaborative relationship with health care providers, payers, and other members of the health care team to optimize interventions.
  • Manages utilization and practice metrics to refine the delivery of care model to maximize clinical, quality, and fiscal outcomes.
  • Completes necessary paperwork and documentation in a timely manner.
  • If order entry in the electronic medical record is part of the job function, completes requisitions/orders per Washington University guidelines.
  • Additional responsibilities include providing proactive care outreach to patients overdue for preventative health services to support value-based care initiatives. Will collaborate with providers, clinical staff, social work, case management, and community resources to assist patients with barriers to care. Responsible for developing and implementing patient centered strategies to improve management of chronic conditions, improve clinical outcomes, and enhance patient satisfaction. 
  • Performs special projects and other duties as assigned.

Preferred Qualifications

  • Triage experience.
  • Ambulatory care management, inpatient case management, or experience in an adult primary care setting. 
  • Knowledge of the concepts and principles of managed care.
  • Interpersonal/human relations skills with ability to relate to diverse groups.
  • Demonstrated leadership qualities including time management skills, verbal and written communication skills, listening skills, problem solving and decision-making skills, critical thinking skills and ability to analyze complex data sets.
  • Strong proficiency in use of personal computer software and databases, including the use of electronic health records and manipulation of statistical data.
  • Supervisory skills.
  • Demonstrated ability to work autonomously and be directly accountable.
  • Demonstrated ability to influence and negotiate individual and group decision making.
  • Demonstrated ability to function effectively in a fluid, dynamic, and rapidly changing environment.
  • Ability to prioritize, delegate, and organize work.
  • Ability to maintain a high level of confidentiality.

Required Qualifications

  • Bachelor’s degree in nursing. 
  • Registered Nurse in the state of Missouri or Illinois. Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).

Salary Range

$59,600.00 - $103,600.00 / Annually

The salary range reflects base salaries paid for positions in a given job grade across the University. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
