Job Overview

The practice coordinator is a vital member of the team who partners with the Deaconess Care Group and Deaconess Health System leadership to engage and educate the physician team, optimize performance on defined key metrics, prepares physician assignments and ensure collaboration with the care team. The Practice Coordinator (PC) will provide support to physicians, advanced practice providers, clinical staff, and clerical staff under the direction of the Practice Administrator (PA) and Director of DCG. The practice coordinator is a vital member of the team who partners with the Deaconess Care Group and Deaconess Health System leadership to engage and educate the physician team, optimize performance on defined key metrics, prepares physician assignments and ensure collaboration with the care team.  They will also be involved in various hospital committees and will present information about the team’s quality data to hospital leadership as requested. While this position does not involve providing direct patient care, they are a part of participating in daily workflows, advocating for patient goals in the care plan, identifying cohorts of patients eligible for specific initiatives, and ensuring communication throughout the care team as the patient moves through their hospital stay. Success in the role will be defined by measurably improved patient outcomes and experience of care, as well as improved utilization of resources.

What You Will Need

  • Associates or Bachelor’s degree from an accredited college or university preferred
  • At least 1 year prior experience in a patient care, customer service or leadership role required.

Certificates, Licenses, Registrations

  • May have the following credentials: Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN)
  • Certified Procedural Coder designation or A.R.T. degree preferred
  • BLS Certified but not required
