The Family Practice & Counseling Network (FPCN) provides comprehensive primary care, dental and behavioral health services. The Network consists of three (3) federally qualified health centers (FQHC) in Philadelphia:   

  • Abbottsford Falls Family Practice & Counseling (Northwest Philly); 
  • Stephen & Sandra Sheller 11th Street Family Health Services of Drexel University (North Philly); and 
  • Health Annex (Southwest). 

Today the FPCN serves more than 20,000 patients annually and is a NCQA certified Patient-Centered Medical Home and recognized by HRSA as a National Quality Leader for Behavioral Health. 


The Network’s mission is to provide person-centered, integrated and comprehensive health services to individuals and families across their lifespan. The Network promotes resiliency and well-being among patients, staff and surrounding communities. Network services are targeted to the surrounding communities and the advisory committee that guides services primarily consists of people from the communities served.


Position Summary

***This is not a 100% remote position***

This position upholds the health center mission to ensure access to recovery-oriented Behavioral Health services and resources that value dignity and worth of the person. This role commits to strong consumer satisfaction and delivery of high quality clinical services that address the whole person, result in better health outcomes, and that help maintain the fiscal and operational capacity of the Behavioral Health Department.


Essential Functions

  1. Psychotherapy – Conduct individual and family therapy sessions , and groups as needed, to meet set productivity target
  2. Charting – Complete all charting as needed and required by contracted funding sources: including intakes, psychosocial assessments, treatment plans, progress notes, service logs, and discharge planning.
  3. Clinical and Administrative Supervision – Participate in regular supervision with designated Clinical and Administrative Supervisors.
  4. Participate in team meetings, case conferences, consultations, and interdisciplinary work groups as scheduled.
  5. Crisis Intervention – Be immediately available to the Family Practice & Counseling staff during working hours in the event of a psychiatric emergency. Participate as a member of the Family Practice & Counseling staff in the event of any other kind of emergency. Participate in regular rotation as On-Call clinician after hours.
  6. Coordination of Care – Coordinate care of clients with the psychiatrist and the Family Practice & Counseling medical staff. Refer clients to other levels of care or other services as needed.
  7. Continued Education – Maintain competencies in mental health and substance abuse as required by licensing and funding bodies. Maintain current PA license to practice.
  8. Member of Family Practice & Counseling’s Multi-disciplinary Team. Collaborates with FPCN Providers and staff to ensure integrated, person-centered care; attend and participate in case conferences, departmental, interdisciplinary, site-level, and network-level team meetings as scheduled.


Essential Functions (Addendum)

1. ADULT THERAPIST: Psychotherapy- Conduct an average of 24 individual and family therapy sessions each week, and groups as needed.

2. CHILD/ADULT THERAPIST: Psychotherapy – Conduct an average of 23 individual and family therapy sessions each week, and groups as needed.

2. CHILD THERAPIST: Psychotherapy – Conduct an average of 22 individual and family therapy sessions each week, and groups as needed.


Requirements

Qualifications

  • Master’s degree in social work, psychology or an applied mental health related field.
  • Minimum of two (2) years’ experience as a therapist and a current license to practice in Pennsylvania as a Licensed Clinical Social Worker, Licensed Professional Counselor, Licensed Marriage and Family Therapist, or Psychologist.
  • Covid vaccination required

Knowledge, Skills and Abilities

  • Experience and competency conducting assessments and psychotherapy with individuals, families, couples, and groups with behavioral health challenges that may include substance abuse.
  • Experience with mental health crisis assessment and intervention.
  • Effective and concise oral and written communication skills and ability to meet deadlines for clinical documentation.
  • Maintains required training in mental health and/or other subspecialties that are a focus of clinical practice, e.g. children.
  • Able to utilize and follow all mental health requirements set forth by applicable state licensing bodies, and the credentialing requirements of insurance companies and other funding sources.
  • Cooperates fully with FPCN’s compliance program.
  • Exhibit behaviors that align with RHD/FPCN’s organizational values, mission, and beliefs and avoid displaying behaviors that are in opposition to those values
  • Support FPCN's commitment to the creation of a trauma-informed system of care that continually recognizes and responds to the impact of traumatic stress on all those who have contact with the organization, including children, adults, families, caregivers, and staff.

Lifting Requirements

vLight: exerting up to 20 pounds of force frequently, and/or negligible amount of force constantly to move objects. The use of arm and/or leg control requires force greater than sedentary, but worker still sits majority of time.

FPCN requires Healthcare Workers to be fully vaccinated. Fully vaccinated means it has been two weeks since the final dose of vaccine. Thus, we require vaccination prior to start for all new hires. This currently does not include boosters.
