This position is responsible for performing clinical assessment of prospective patients and providing oversight of RN Care Managers within various programs. This includes performing initial visits with prospective patients, assisting with resolution of patient escalations and interfacing with internal and external care team members, patients, and patient caregivers as appropriate. Must be licensed in the state of Georgia.

Performs initial virtual, comprehensive assessments of patients; for those who are clinically eligible, determines which devices are most appropriate for each patient.
Ensures documentation supports administrative functions such as quality management and billing.
Oversees the monitoring of patient vitals, symptoms, and labs in collaboration with a team of RN Care Managers and Medical Directors.
Responds to patient escalations identified within virtual programs, i.e. abnormal or outlier vitals, symptoms, or other complexities related to patient care.
Demonstrates a professional attitude, sound judgment, and empathy in all interactions with patients, caregivers, and other internal/external healthcare team members.
Participate in other projects or duties as assigned.

Graduate degree in nursing required (MSN, DNP) or a post-graduate certificate in nursing field (for RNs).
Compact State Licensure required (RN Compact).
Ten (10) plus years of experience required preferably in acute or ambulatory care.
Experienced in EHR/CMR systems.
Experience working successfully in a fully virtual environment.
Experience working with the senior population in home health, clinics, or other health service setting a plus.
Or equivalent combination of education and/or experience.
Proficient in nursing and associated practice procedures.
Demonstrates excellent clinical insight.
Exhibits empathy, patience, and a strong dedication to providing exceptional patient care.
Possesses outstanding written, verbal, and interpersonal communication abilities.
Maintains a highly personable and engaging telephone demeanor.
Capable of working effectively with minimal supervision.
Collaborates effectively within a team environment.
Capacity and readiness to broaden state licensure to accommodate emerging markets and collaborators.
Able to invoice both governmental and commercial payors, enabling the assignment of benefits to the organization.
Salary: $92,500-$124,900
This role is bonus eligible based on individual and company performance.
