Summary of Position
The Manager, Care Coordination works closely with leadership to facilitate the delivery of outstanding service and clinical excellence to Teladoc Health members. Overseeing the supervisors of Care Coordination and Referral Coordination, along with their teams, the manager is responsible for leading the day-to-day clinical operations of the team, ensuring the delivery of high-quality, comprehensive, and patient-centered care. This role involves managing staff, integrating services, and fostering collaboration among internal and external healthcare providers and partners.
Essential Duties and Responsibilities
Manage program planning, implementation and monitoring across people, process, technology and projects including facilitation and coordination with clinical operations, clinical support, population health, value-based care.
- Oversee and manage the performance of the Referral Coordination, Medical Assistant and Nurse Care Coordination teams
- On-boarding, leading, motivating and developing the Care Coordination team to cultivate a positive and productive workplace environment.
- Monitoring ongoing performance metrics and quality of all team members, managing the staff to meet or exceed productivity and quality metrics.
- Developing and supporting ongoing training activities to ensure staff is fully supported to deliver service excellence within each professional scope of practice.
- Ensuring superior member and client experience which may include communicating directly with members, internal partners or external partners via written, telephone or video communication.
- Leading the team to meet all service level commitments and turnaround times
- Optimize Care Coordination team processes and efficiency
- Identify opportunities for program improvement and lead the definition, development, implementation, evaluation and reporting of designated program standards and initiatives.
- Lead and oversee implementation of related organizational policies and procedures.
- Collaborate with internal partners, including program, product and development to build and maintain accurate, complete and current documentation tools.
- Meet practice, client and member needs through strategic planning and data collection/analysis
- Collect, analyze, and interpret data to identify trends, generate insights, and implement process improvement to optimize workflows across the continuum of care
- Collaborate with partners in Clinical Operations, Population Health, and Integrated Care Delivery to ensure the highest quality, cost effective care is delivered.
- Ensure program activities are coordinated, aligned and sequenced with other organization strategic objectives and/or other improvement activities.
- Continually assess external environment and emergence of value-based initiatives, assessing the potential implications and opportunities for Teladoc Health.
- Monitor the performance of patience care delivery systems by data driven sets of metrics

Qualifications Required for Position
- Bachelor’s degree in Nursing
- Current license as an RN in good standing with unrestricted ability to hold an RN license in all 50 US States.
- 5+ years of people management experience.
- 5+ years of clinical experience in a healthcare setting
- Proficiency in using technology and software programs including Microsoft Word, Excel, Outlook.
- Skilled in organizing and prioritizing work in a fast-paced environment.
- Motivation to meet both personal and team deadlines.
Qualifications Preferred for Position
- Certification in Care Coordination and Transition Management Certification (CCCTM) or Certified Professional Patient Navigation (CPPN) or equivalent
- Experience leading other people leaders
- Outstanding team player & strong interpersonal skills.
- Excellent written and verbal communication skills.
The base salary range for this position is $95,000- $105,000. In addition to a base salary, this position is eligible for a performance bonus and benefits (subject to eligibility requirements) listed here Teladoc Health Benefits 2025. Total compensation is based on several factors including, but not limited to, type of position, location, education level, work experience, and certifications. This information is applicable for all full-time positions.
