Overview:
THIS WORK MATTERS!
This specific position will not have a designated caseload but will provide housing and rental assistance for children with intellectual and/or developmental disabilities and their families. The purpose of case management is to access, coordinate, and assure the delivery of services and supports required by individuals with intellectual and/or developmental disabilities. Case Management will assist eligible individuals under their individualized plan in gaining access to needed medical, social, educational, and other services. This is a limited-duration position. Limited-duration positions can last up to two(2) years, but can end at any time. Must be fluent in Spanish (speak, read, and write). Language testing is required.
As the Housing Case Manager LD, other duties will include:
Assist families of children eligible for Multnomah County IDD services with applying for housing assistance
Assist families with filing out applicable housing forms, monitoring the process to ensure timely submissions to the housing funding team
Report possible protective services issues and provide initial information to assist the investigator
Maintain case records and document actions taken in accordance with federal, state, local policy
Develop, submit, and modify necessary documentation to implement/change/adjust payments and program benefits
Secure the needed resources to stabilize the situation of the client with developmental disabilities
Ensure that client is eligible for services to which they are entitled and receives the support that adequately meets their needs

Minimum Qualifications/Transferable Skills*:
Education/Training:
High School Diploma or GED
Two (2) years experience of paraprofessional level providing assistance and services to human services/Intellectual Development Disabilities services
Ability to promote a culture of respect, inclusiveness, and appreciation of diversity perspectives, backgrounds, and values
Must pass a criminal background check
Other:
Knowledge, Skills, Ability (KSA) Must be fluent in Spanish (speak, read, and write) Testing is required.

Routine Telework: This position is designated as “routine telework,” meaning you will be working remotely (from home); a hybrid schedule also is available. As with all routine telework positions, employees may be required to come into the office on occasion. The designation of Routine Telework may be subject to change at a future time. All employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations. The onsite work location for this position is Five Oak Building, 209 SW 4th Ave.
