Overview:

THIS WORK MATTERS!

The Case Manager 1 (CM1) provides assistance to IDDSD Housing Team staff who serve the Intellectual and Developmental Disabilities populations in Multnimah County. Functions performed by the CM1 will include administrative tasks to support the delivery of the Short-Term Rent Assistance program and the Shelter Plus Care grant. This is a Limited Duration position. Limited-duration positions can last up to two (2) years, but can end at any time. 

As the Limited Duration Case Manager 1, other duties will include:

  • Review Short Term Rent Assistance (STRA) funding requests submitted to the IDDSD Housing Team, screen for eligibility, issue approvals and denials per program policies and funder requirements.

  • Request and review required documentation per STRA funding requirements and process support payments per DCHS Accounts Payable criteria.

  • Process recertification documents for Shelter Plus Care participants

  • Obtain relevant training to access and complete data entry steps for IDD Housing Assistance programs in the HMIS system

  • Complete data entry related to Short-Term Rent Assistance services and participation in the Shelter Plus Care grant, provided to the IDDSD services populations

  • Provide housing case consultation to co-workers and staff of partner programs/agencies and provide requested housing resource information

  • Maintain current knowledge on all policies and procedures for the program administered

Minimum Qualifications/Transferable Skills*:

Education/Training:

  • High school diploma or GED 

  • Two (2) years of paraprofessional-level experience providing assistance and services to human services/social service clients, including individuals with Intellectual Developmental Disabilities

  • Ability to promote a culture of respect, inclusiveness, and appreciation of diverse perspective, backgrounds, and values

  • Must pass a criminal background check
