We’re seeking a compassionate, holistically-inclined behavioral health professional to join our team as the LGBTQIA2S+ Mental Health Consultant - Intensive Care Coordinator. The Intensive Care Coordinator (ICC) team is a compassionate and dedicated group of professionals who provide trauma-informed, culturally responsive care coordination for adults with complex behavioral health challenges. Our mission is to develop personalized care plans and facilitate community-based support, guiding individuals toward a healthier and more fulfilling life. In this role, you will provide client-centered, trauma-informed care coordination and personalized care plans for adults with complex behavioral health needs, including severe emotional disturbances, substance abuse, and/or criminal behavior. You will also assist clients in navigating the behavioral health system, facilitate coordinated care with system partners, and provide clinical assessments to eligible LGBTQIA2S+ individuals with complex behavioral health needs. Additionally, you will coordinate specific case plans to meet the needs of disadvantaged clients and their families, ensuring culturally specific, client-centered, trauma-informed care.

Knowledge, Skills and Abilities: This position requires extensive knowledge of and experience working with the LGBTQIA2S+ community, as well as broad knowledge of culturally matching community resources. This role provides outreach, engagement, and care coordination to help LGBTQIA2S+ youth and families overcome barriers to health care. The role also requires an in-depth understanding of how to support the needs of LGBTQIA2S+ people with a multitude of intersecting identities (ex: BIPOC, immigrant/refugee, differently-abled, etc). This is demonstrated by knowledge of cultural norms within the LGBTQIA2S+ community, as well as having a close affiliation with and lived experience in the LGBTQIA2S+ community.
 

Teleworking: At this time, this position is designated to “routine telework,” meaning you will be working remotely (from home and community) most of the time; a hybrid schedule also is available. As with all routine telework positions, employees may be required to come into the office on occasion. The designation of Routine Telework may be subject to change at a future time. Currently, all employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations.

As the LGBTQIA2S+ Mental Health Consultant - Intensive Care Coordinator you will:

Care Coordination 

  • Coordinate and consult with judicial, health, correctional, intellectual and developmental disabilities, and mental health service providers.

  • Address barriers to access, collaborate with service providers, and monitor client progress.

  • Develop Intensive Care Coordination plans to align systems, identify client strengths and needs, and refer clients to appropriate mental health and other support services.

  • Provide compassionate, empathetic, culturally responsive care coordination and case management for LGBTQIA2S+ clients with complex behavioral health needs.

Documentation and Compliance

  • Maintain timely and accurate documentation for each member.

  • Track and enter data for program reporting and evaluation.

  • Utilize Hospital System Electronic Medical Record, CareOregon Electronic Care Management System and other relevant systems to gather comprehensive client information.

  • Assist with intake duties and assign clients to care coordinators as needed.

Assessment, Care Planning and Interdisciplinary Team Meetings

  • Assess and identify member's care coordination needs through member contact, clinical and utilization information review, and provider/community partner collaboration.

  • Identify risk factors, service needs, and support needs.

  • Develop, review, and update individualized care plans based on needs assessments.

  • Support collaboration of providers and community partners in implementing care plans.

  • Establish and facilitate monthly Interdisciplinary Team (IDT) meetings.

  • Ensure cultural/racial/ethnic factors are considered in the assessment and screening process.

Community Partnerships

  • Build and maintain relationships with community partners, including hospitals, outpatient clinics, specialty service providers, and Health Share of Oregon plan partners.

  • Collaborate with community partners to meet the behavioral health needs of LGBTQIA2S+ clients.

  • Demonstrate cultural sensitivity and awareness in working with LGBTQIA2S+ clients and community partners.

 TO QUALIFY:

Required Minimum Qualifications/ Transferable Skills*:

Education/Certification: Masters Degree; must meet the standards of Qualified Mental Health Professional - (QMHP-C) with the Mental Health & Addiction Certification Board of Oregon (MHACBO) - more information can be found at MHACBO website. Current QMHP-C and/or licensed Board of Oregon (LPC, LCSW, PHD,etc) (Required)

  • Graduate degree in psychology or

  • Graduate degree in social work or counseling or

  • Graduate degree in a behavioral science field, AND

Experience: Two (2) years of professional work experience as a counselor, Mental Health treatment professional, social worker, or therapist working with individuals suffering from substance use disorder (SUD).

Other: 

  • Valid Driver’s License

  • This position requires a Background Investigation, which may include being fingerprinted.
