Accolade is currently Recruiting for a Full Time Health Coach I.
This position will be remote for applicants who are not close to our area offices. The position will be hybrid for applicants who are close to our area offices.
The hours for this role can be either 10:00am to 6:30pm EST or 10:30am to 7:00pm EST.
This position uses multiple platforms, strong critical thinking skills, problem solving skills, and high call volume.
Applicants who have experience in strong customer service support, medical customer service, retail, food industry (barista/servers), with strong de-escalation skills along with solving a problem and seeing it through, this position is for you!
Role overview
As a Health Coach I , who reports to the Health Coach Supervisor, is responsible for establishing trusting, supportive, collaborative relationships with patients and their families. Health Coaches build relationships with patients in a clinical setting, working alongside medical providers, other health coaches, and behavioral health specialists in a collaborative, empathetic team.
The Health Coach is at the helm of patient engagement, helping patients meet preventive, chronic, and acute healthcare needs alongside other key members of a collaborative Accolade Care team. Health Coaches foster a relationship with patients that meets them where they are at with their health needs, allowing them to be active participants in their healthcare journey while striving to promote healthy lifestyle choices and behaviors.
Our patients will have access to a dedicated Health Coach, Registered Nurse, "virtual first" primary care provider and/or virtual therapist. Each member of our team plays a vital role in keeping our patients happy and healthy. Our patients are more than the symptoms that they present to us with. We aim to understand their ultimate goals in life, set realistic care plans in place, and assist our patients in accomplishing them. Together we are stronger.
A day in the life…
- Establish trusting relationships with patients
- Accurately and confidently communicate with patients to help them take ownership of better managing their health
- Communicate with patients across our platforms via in-app messaging, phone, and email and escalating patient needs to members of the AccoladeCare team as needed
- Help patients understand the rationales behind proper management of their acute/chronic conditions and how they can manage them
- Actively steer members to virtual first option, with expectation of directing to local in-person care as appropriate
- Uncover barriers for patients that prevent them from obtaining needed care and work with the patient to address/overcome.
- Accountable (in partnership with providers and AccoladeCare team) for continuity of care for patients pre and post vPCP visit additional monitoring, follow-up, and care coordination
- Gain familiarity with chronic health conditions pertinent to targeted patient populations
- Provide patient education on chronic disease management and collaboration with AccoladeCare nurse
- Support the management of chronic conditions within the Health Coach scope
- Support the PCP/provider of care in ensuring pertinent information is available to support a holistic patient encounter
- Outbound calls to targeted eligible patients to engage them in being an active participant in their healthcare
- Assist with the coordination of care across the continuum, such as: helping patients schedule their vPCP and/or in-person PCP appointment, assisting in the coordination of referrals to specialists or facilities for additional testing, and follow up on identified care plan items or referral results back to patients vPCP
- Learn core insurance benefits and basic claims knowledge to facilitate care navigation
- Present on areas of interest/expertise in health and wellness to team members and patients
- Contribute to open team forums on problems and process improvements by listening, sharing input ideas, enacting decisions and plans
- Help patients reduce and/or eliminate avoidable admits, Emergency Room usage & readmissions
- Help patients avoid inappropriate health care usage and process errors
What we are looking for…
- Health Coach certification is preferred
- Should have prior experience in a service environment
- Some clinical experience is preferred, i.e. EMT, MA, or CNA
- Strong interpersonal communication skills with exceptional listening abilities required
- Compassionate, kind and culturally competent
- High level of working in collaboration
- Display ability to work in multiple non integrated platforms
- Strong written communication skills required
Hiring Range
$21.2-$25.35Hourly
Actual compensation packages within that range are based on a wide array of factors unique to each candidate, including but not limited to skill set, years and depth of experience, certifications, and specific location.
