“Advance Directive” is an umbrella term for documents which declare a person’s preferences and values. They are used to safeguard wishes for health care and finance management at the end of life and when incapacitated. Advance directive documents include the following:
Living Will: setting out a patient’s treatment and care preferences to physicians when people are unable to communicate
Power of Attorney for Healthcare: designating a person to make medical decisions if a person is incapacitated
Power of Attorney for Finance: designating a person to oversee property and money for the person’s benefit
Hospital Visitation Authorization/Declaration: lists people authorized to visit (or prohibited from visiting) during a hospital stay
With guidance, the position will draft the process to complete each of these forms, and create easy-to-understand educational 2-pagers on topics such as: Why do I need an Advance Directive?, FAQ about Advance Directives (What power do I give up when I complete these forms? What do I do with the forms after completing them? What happens if I move to a different state?)
