 Humana Healthy Horizons in Virginia is seeking an Early Intervention (EI) Coordinator (Care Management Support Professional 2) who will contribute to administration of care management. They will provide non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and well-being of members ages 0-3 requiring early intervention services. The EI Coordinator’s work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

  • Oversees all early intervention activities for the health plan.
  • Maintains up-to-date knowledge about early intervention services and related federal and Commonwealth laws.
  • Serves as a point of contact with stakeholders and providers serving early intervention members.
  • Responsible for utilizing reports and other resources for identifying member gaps in developmental screenings and developing plan approaches for improving screening rates.
  • Works with Care Managers to link identified members with early intervention services.
  • Collaborates with the Care Management team and with EI providers regarding the member’s Individualized Family Service Plans (IFSP).
  • Develops transition plans for members who will be aging out of EI services before their 3rd birthday.
  • Performs other duties as required by health plan.

Required Qualifications

  • Must reside in the Commonwealth of Virginia or within a 40-mile radius from Virginia in a bordering state/district (Washington DC, MD, WV, KY, TN & NC).
  • Bachelor’s degree in health or human services field OR an active Licensed Practical Nurse (LPN) licensed in the Commonwealth of Virginia.
  • One (1) year experience serving Early Intervention populations and knowledge of EI services within the Commonwealth of Virginia (e.g. Pediatric office, MCO’s, school/public).  
  • Must have knowledge and skills in Infant and Toddler development.
  • Intermediate working knowledge using Microsoft Office Programs specifically Teams, Excel, PowerPoint, Outlook and Word.
  • Exceptional communication & interpersonal skills with the ability to build rapport with internal and external customers and stakeholders.
  • Decision making skills regarding own work approach/priorities, and work assignments, standards and resources.
  • Ability to manage multiple competing priorities in a very fast-paced environment.
  • Strong understanding and respect of all cultures and demographic diversity.
  • Strong written communication skills and a strong advocate for members at all levels of care.
