Job Description

The Director, Clinical Program Management will work directly with national and market teams to develop and scale Privia’s clinical programs, particularly with regard to medication adherence and care coordination. They will be responsible for meticulously managing the tactical components of design in partnership with clinical and administrative thought leaders across the organization with the goal of achieving quality, cost and patient experience goals under our value-based contracts.

 

  • Collaborate with national departments and market leaders to project manage, design, implement, monitor, evaluate and improve the performance of Clinical Programs that improve performance of Value Based Contracts.

  • Collaborate with market and provider leaders to monitor and evaluate the effectiveness of value-based care programs, identifying areas for improvement, with a particular focus on measures related to medication adherence and care coordination. 

  • Collaborate with vendors, technology teams, and market leaders to enable seamless care coordination across all sites of care. Drive initiatives to improve contract quality measures, including readmissions, medication reconciliation, and transitional care management. 

  • Build and maintain corporate expertise on medication adherence strategies. Partner with market leaders to test and evaluate various medication adherence approaches. Use data to identify and promote medication adherence strategies that demonstrably improve contract quality compliance rates. 

  • Collaborate with analytics teams to track and analyze near-term and long-term outcomes (including ROI) and quality metrics. Utilize data insights to identify trends, opportunities for improvement, and to inform tactical decision-making alongside market and national teams. 

  • Maintain a close working relationship with the National Director, Contract Quality to ensure a comprehensive understanding of quality contract performance. Proactively identify and address potential quality contract performance gaps. Ensure that initiatives and processes are aligned with contract requirements. 

  • Document repeatable processes and best practices in the form of Standard Operating Procedures (SOPs). Promote the use of standardized processes to ensure consistency and efficiency across the organization. Drive continuous quality improvement efforts.

Qualifications

  • Nursing, Social work, pharmacy, medical, or similar clinical expertise preferred.

  • 5+ years of experience in a clinical management role

  • Familiarity with reimbursement models such as pay-for-performance, shared risk, and full capitation and strong understanding of value-based care tenets and tactics

  • Highly collaborative with, and credible among, multiple stakeholders; including clinical, administrative and analytic teams

  • Analytical and comfortable with numbers – will use ROI analyses to drive decisions

  • Strong project management skillset 

  • Must comply with HIPAA rules and regulations

The salary range for this role is $127,000.00 - $149,000.00 in base pay and exclusive of any bonuses or benefits (medical, dental, vision, life, and pet insurance, 401K, paid time off, and other wellness programs). This role is also eligible for an annual bonus targeted at 15% and restricted stock units. The base pay offered will be determined based on relevant factors such as experience, education, and geographic location.
