Criminal Justice Liaison-Northern Indiana- Mobile

Job Summary:
Community Justice Liaison is responsible for engaging with members prior to their release from incarceration and during the transition period as these members re-enter their community. The Liaison will assist in delivering a coordinated re-entry experience for our members through collaboration with the Department of Correction and internal and external stakeholders. The Liaison will help ensure end-to-end care coordination by engaging with incarcerated members prior to release, obtaining information on the physical, behavioral, and social needs of the member to assist them with goals and resources for success.
Essential Functions:
- Provide in-reach education and engagement services to incarcerated individuals and staff in the Indiana Department of Correction. Engage with the member in a variety of settings to establish an effective, professional relationship. Settings for engagement may include but are not limited toa correctional facility, hospital, provider office, community agency, telephonic or electronic communication
- Responsible for interfacing with the existing care coordination continuum, including the member, Indiana Department of Correction, CareSource care management and Life Services staff, community health partners and community agencies to foster the transition of the member back into their community. Ensure the creation of a care plan and help coordinate the connection of the member to community providers and resources
- Maintain appropriate documentation
- Responsible for gathering member data
- Participate in relevant governmental and advocacy meetings
- Contribute to operational and strategic feedback to programmatic development
- Responsible to serve as an interdepartmental and inter-market liaison to gather/share information about issues and barriers related to the successful execution of our program
- Help develop and deliver training and training materials related to Indiana Re-entry Program
- Act as a liaison to government agencies and social service/community resources to communicate ideas, address concerns and relay solutions
- Educate members through implementation of tools, processes, and competencies
- Perform any other job-related duties as assigned
Education and Experience:
- Bachelor’s degree in health care, business or related field or equivalent years of relevant work experience is required
- A minimum of two (2) years of care coordination or related experience is required
- Experience working with community justice population preferred

Licensure and Certification:
- Re-entry Specialist Certification, or equivalent approved training program, is preferred
- Employment in this position is conditional pending successful clearance of a driver’s license record check. If the driver’s license record results are unacceptable, the offer will be withdrawn or, if employee has started employment in position, employment in this position will be terminated
- To help protect our employees, members, and the communities we serve from acquiring communicable diseases, Influenza vaccination is a requirement of this position. CareSource requires annual proof of Influenza vaccination for designated positions during Influenza season (October 1 – March 31) as a condition of continued employment. Employees hired during Influenza season will have thirty (30) days from their hire date to complete the required vaccination and have record of immunization verified.

Working Conditions:
- Mobile Worker: This is a mobile position, meaning that regular travel to different work locations is essential. Will be exposed to weather conditions typical of the location and may be required to stand and/or sit for long periods of time.
- May be required to travel greater than 50% of time to perform work duties. A valid driver’s license, car, and insurance are necessary for work related travel
- Required to use general office equipment, such as a telephone, photocopier, fax machine, and personal computer
- Flexible hours, including possible evenings and/or weekends as needed to serve the needs of our members and may refer members to other CareSource resources
Compensation Range:
$46,500.00 - $74,500.00CareSource takes into consideration a combination of a candidate’s education, training, and experience as well as the position’s scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level.
