Job SummaryThis position is responsible in a clinical advisory role to assess members for Wellness Education and Disease Management; introduce members to utilize the Personal Health Manager website as a tool for members to better manage their health; educate members regarding Wellness and specific condition; facilitate coordination of care for members identified through Claims; completion of the Health Risk Assessment (HRA); and self-referral/referral from the Utilization Management (UM) and/or Case Management (CM) departments, or physician referrals. If a Registered Nurse (RN), may respond to web based Ask A Nurse questions.*Hospital experience needed.
*Must be willing and able to attend occasional Wellness events with our members.
JOB REQUIREMENTS:
- Current, unrestricted license in the state of operations in one of the following: Registered Nurse (RN), or Licensed Professional Counselor (LPC), Registered Dietician (RD) or Licensed Master Social Worker (LMSW).
- 3 years clinical (hospital) experience.
- Patient education experience.
- Computer/system software experience including Web.
- Verbal and written communication skills; analytical skills; sound clinical judgment.
- Incumbents with nursing licenses in positions/departments requiring multi-state licenses are required to obtain and maintain additional current, valid, and unrestricted applicable nursing licenses in other states as determined by management. Multi-state license fees will be provided by HCSC. Incumbents with other clinical licenses are not required to obtain multi-state licenses.
The Compensation/Salary offered for this Position will vary depending on Job-related skills, education, knowledge and experience. This role aligns with an Annual Performance Incentive (Bonus) plan.
