About the job

The Clinical Consultant is responsible for Clinical Oversight of ECM Lead Care Managers to address the client's medical and behavioral health needs, as well as social determinants of health. The Clinical Consultant will communicate directly with ECM Lead Care Managers and occasionally with patients via telephone, telehealth, or during Interdisciplinary Care Team meetings (as needed), to discuss medication management/adherence, ensure appropriate follow up with Primary Care Provider, Behavioral Health Provider and other specialist(s). They will provide guidance during any emergency situations encountered by ECM Lead Care Managers. The Clinical Consultant will collaborate with the client’s broader care team (PCP, BH clinicians, etc.) to provide client advocacy and ensure that the clients are receiving needed services and measuring progress towards the goals outlined in their patient-centered individualized care plan.

What you’ll do

Remote duties performed through HIPAA-compliant hardware and software:

  • Provides oversight of ECM Individual Care Plans and determines if Client’s Care Management needs are sufficient to be managed by an ECM Lead Care Manager or require the Consultant’s intervention in the form of recommendations to implement in their Care Plan
  • Participates in developing patient-centered care plans for the clients on their panel. Approves initial plan and biannual updates
  • Consults with Lead Care Managers to review client medical visit summaries, discharge summaries, and clinical care coordination needs
  • Provides health and preventive care education for acute health conditions, chronic disease management, and medication monitoring
  • Engages vulnerable populations as part of a multidisciplinary outreach team as needed
  • Helps to expand programming that addresses SDoH and enhances connections to community-based organizations
  • Works with hospitals to coordinate hospital admission/discharge plans with the behavioral health clinician, PCP, pertinent specialists and other organizations with the goal of preventing readmission, if possible
  • Performs timely medication reconciliation following transitions in care. Supports medication adherence
  • Implements prevention & engagement activities
Your experience and qualifications
  • Willing and able to work Monday-Friday 8:30am-5:00pm in Pacific time zone, with the possibility of afternoons, evenings, and/or weekends
  • RN and/or LCSW licensed in California, with the ability to obtain additional state licensure as needed
  • Proven, results-driven business leader with experience in rapid-growth organizations
  • Two to three years of care management experience
  • Must possess strong organizational skills
  • Ability to work independently
  • Proficient with Microsoft and Google applications
  • Strong communication skills
  • Strong motivational skills including conflict/dispute resolution techniques
  • Must have access to reliable internet connection

Compensation and benefits:

  • $75,000- $105,000 annually: Pay for this position will be based on several factors, including, but not limited to: your prior experience and skills related to the position, geographic location, company needs, and current market demands.
