Acentra seeks a Care Navigator [Personal Health Consultant] (Remote in Illinois, preferred) to join our growing team.

Job Summary:

The Care Navigator:

  • Assists members in accessing the right care at the right time by building strong relationships that support member engagement in their medical and/or behavioral health care treatment plans.
  • Are committed to removing the member’s barriers to care by identifying needed resources, helping them navigate through health care services and systems, promoting holistic health through education, and reducing social barriers. The Care Navigator helps members to optimize the use of their benefit plan through steerage to in-network providers and work closely with the Clinical Care Team, which may include but not be limited to doctors, nurses, and other clinical staff to support positive client health outcomes.

** This role is Remote within Illinois, preferred. **

** This role is contingent upon being awarded a contract. Start dates and final offers are contingent upon the contract award and final contract start dates. **

Job Responsibilities:

  • Outreach to members to assist them in meeting their preventative health needs.
  • Complete health and social needs assessment to assist in the development of individualized care plans.
  • Work within a patient’s benefit network for the best outcome and to enhance access to care.
  • Potentially cross-trains to support other contracts within Acentra Health to provide a flexible workforce and meet client/customer needs.

Required Qualifications/Experience:

  • Bachelor’s degree in a health-related field (e.g., Social Work, Counseling, Psychology, or another closely related healthcare field) with 1+ years of experience in medical, mental health, and/or behavioral healthcare, …
