Join our team and make a positive impact on the lives of others! We are looking for a Care Management Trainer for our Care Management team. 

This is a remote position with potential travel into the FreedomCare hub closest to you for meetings and/or collaboration.

Department & Position Overview: 

The Health Home Care Manager Trainer is responsible for delivering comprehensive training programs to newly hired Health Home Care Managers. This role focuses on equipping individuals with the necessary skills and knowledge to effectively manage patient care.  

 The Trainer will design, implement, and evaluate training curricula, ensuring all Care Managers are proficient in the required competencies and compliant with NYS Health Home regulations. Training topics will include client care, documentation requirements, as well as NYS, lead, and company policies and procedures.   

Every Day You Will:

  • Design and develop training materials and curricula that cover all aspects of health home care management, including patient assessment, care planning, and coordination of services. 
  • Conduct training sessions for newly hired Care Managers, using a variety of instructional techniques and formats such as simulations, team exercises, group discussions, videos, and lectures. 
  • Conduct routine training sessions for Care Managers and provide additional training when specific needs are identified to ensure ongoing competency and compliance with best practices. 
  • Provide personalized coaching and support to trainees, addressing individual learning needs and ensuring comprehensive understanding of care management practices. 
  • Assess trainees' progress through evaluations and feedback, identifying areas for improvement and providing additional training as necessary. 
  • Continuously evaluate the effectiveness of training programs, making improvements based on feedback and the evolving needs of the health home care management field. 
  • Work closely with Care Manager Specialists, supervisors, and other stakeholders to ensure training content is relevant, up-to-date, and aligned with organizational goals and regulatory requirements. 
  • Maintain accurate records of training sessions, attendance, and trainee progress. 

Ideal Candidate Will Possess:

  • Bachelor's Degree is required
  • A minimum of 2-3 years of experience in training/development with strong proficiency in facilitation, design and implementation of learning, preferably within a healthcare setting. Prior experience as a Care Manager or in a similar role is highly desirable.  
  • In-depth understanding of health home care management, patient care coordination, and NYS Health Home regulations. 
  • Excellent verbal and written communication skills, with the ability to explain complex concepts in a clear and understandable manner. 
  • Strong interpersonal skills, with the ability to build rapport and effectively mentor and motivate trainees. 
  • Exceptional organizational skills, with the ability to manage multiple training sessions and maintain detailed records. 

Nice-to-Haves:

  • 3 + years of health homes experience 
  • Master's Degree

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $20.00 and $27.00 an hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
