Join our team and make a positive impact on the lives of others! We are looking for a Care Management Referral Specialist for our Care Management team. 

This is a remote position with potential travel into the FreedomCare hub closest to you for meetings and/or collaboration.

Department & Position Overview: 

The Care Management Referral Specialist is a pivotal role responsible for managing patient referrals, ensuring effective communication with referral sources, and supporting Care Managers throughout the intake process.  

The Specialist will handle referrals from various sources, contact patients to assess their needs, and explain the benefits of the care management program. Utilizing their strong communication to influence and persuade, they will arrange enrollment visits with Care Managers. 

 Additionally, the Specialist will maintain open communication with referral sources, providing updates on referral statuses and seeking assistance in reaching patients when necessary. Building strong relationships with both patients and referral sources is essential to converting referrals to active cases. The role also involves participating in community marketing initiatives to promote Care Management services. 

Every Day You Will:

  • Receive and manage patient referrals from various sources, conducting initial calls to review their needs and the reasons for their referral. Using sales expertise to effectively communicate the value and benefits of the care management program the referral specialist will arrange enrollment appointments with care managers. 
  • Serve as a direct point-of-contact for referral sources and ensure seamless communication, providing regular updates to referral sources on the status of referrals. 
  • Collaborate with referral sources to help reach patients when direct contact is challenging and build strong relationships with patients and referral sources to convert referrals to active cases. 
  • Support care managers in the intake process by contacting and reaching out to patients for the rescheduling of cancelled or missed visits. 
  • Track and document all referral activities in the Care Management platforms, maintaining accurate and detailed records of patient interactions and referral statuses. 
  • Participate in community marketing efforts and assist with developing and implementing marketing strategies to increase patient referrals and program awareness. 
  • Maintain a conversion rate above 80% for referrals to active cases, and continuously evaluate and improve referral processes to enhance efficiency and patient satisfaction. 
  • Ensure compliance with NYS Health Home and lead Health Home regulations. 

Ideal Candidate Will Possess:

  • 1+ year of sales experience in a healthcare setting preferred or experience interacting with others in order to drive a specific outcome. 
  • Ability to connect and engage with others and develop rapport quickly connect with people over the phone and foster trusting and warm relationships. 
  • Ability to handle multiple tasks and work in a fast-paced environment. 
  • Strong organizational skills and attention to detail. 
  • Familiarity with data management systems and documentation practices. 

Nice-to-Haves:

  • Bilingual Spanish preferred.
  • Experience working with health homes, HARP or any other related Medicaid program

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $18.00 and $23.00 an hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
