Overview
Care Facilitator (Fully Remote)
*Part Time Position Available Immediately
*Available Schedule: Saturday & Sunday 8a-4p CST (9a-5p EST)
The Starting Point Care Facilitator plays a crucial role in coordinating and managing the care of patients. They ensure effective communication, continuity of care, and optimal patient outcomes. They work closely with healthcare providers, patients, and their families to facilitate a patient-centered approach to healthcare delivery by fielding triage calls and coordinating care across services.
Responsibilities
Patient Engagement:
- Respond to incoming patient and family questions and escalate emergent triage calls to the clinical team
- Provide timely, accurate, and helpful information to patients and support the overall care goals of our programs by proactively contacting patients for longitudinal care management
Treatment Coordination:
- Coordinate and schedule appointments and referrals to ensure timely and appropriate follow-up care
Communication and Collaboration:
- Facilitate effective communication and collaboration among healthcare providers, patients, and their families
- Serve as a liaison between services and external providers to ensure thorough care coordination
Monitoring and Evaluation:
- Monitor patients' progress and response to treatment, identifying any changes or complications
- Communicate escalations or new health risks to clinical leadership
Documentation and Record-Keeping:
- Maintain accurate and up-to-date communication logs. Adhere to documentation standards and regulatory requirements
Quality Improvement:
- Participate in quality improvement initiatives and performance improvement projects
- Identify areas for improvement in care coordination, patient outcomes, and patient satisfaction
- Implement strategies to enhance the quality of care
Compliance and Regulations:
- Adheres to HIPAA regulations and maintain patient confidentiality
- Ensures compliance with clinical guidelines and standards of care
Qualifications
- Some College or associate degree in clinical sciences, Nursing, or a related field
- Minimum two (2) years of experience collecting referral information in the healthcare market
- Medical Assistant certification preferred, but not required
- Experience with electronic medical record software and proficient use of the Microsoft Office suite
- Ability to multitask, prioritize, and manage multiple projects simultaneously
- Excellent written and verbal communication skills and ability to work independently or in a team setting
