Acentra seeks a Care Coordinator to join our growing team.

Job Summary:

Provide telephonic care coordination outreach to support preventative health activities, including social needs, to reduce barriers to access and care while optimizing health outcomes.

Job Responsibilities:

  • Ensures the responsible delivery of comprehensive care coordination services to enrollees. 
  • Conducts general assessments for appropriate referrals to Personal Health Consultants, primary health plans, and other resources.
  • Ability to provide critical resources to members in real-time while also transitioning members to Personal Health Consultants for ongoing needs.
  • Able to communicate effectively with members of all ages and able to communicate via an interpreter when necessary.
  • Maintain metrics for telephonic outreach and member engagement.
  • Participates in team case reviews for collaborative assessment and coordination planning to ensure quality care as needed. 
  • Assists the Lead, Supervisor, and/or Manager in ensuring the cost-effective and efficient achievement of contract requirements, including service delivery, by reporting accurate and timely work hours incurred during position duties.

*This role is contingent upon being awarded a contract. Start dates and final offers are contingent upon the contract award and final contract start dates.

Requirements

Required Qualifications/Experience:

  • Associate's degree in a health-related field and/or medical assistant training (or higher).
  • 2+ years of experience in care coordination (field or clinical environment preferred).
  • Telephonic outreach or call center experience.
  • Knowledge of customer service principles.
  • Knowledge of medical terminology.
  • Strong written and verbal communication skills.
  • Strong field or motivational interviewing skills.
  • Strong computer skills (e.g., care management applications, Internet/Web, Microsoft Office [Word, Excel, Access]).
  • Strong prioritization and organizational skills.
  • Ability to receive verbal and written feedback in a professional manner and implement performance and productivity improvements as needed.
  • Ability to interpret real-time and historical information to inform and assist members in real time.

Preferred Qualifications/Experience:

  • Bachelor’s degree in a health-related field.
  • 3+ years of current experience in care coordination (field environment).
  • Knowledge of social needs.
  • Knowledge of regulatory and accreditation standards.
  • Knowledge of principles, ethics, and precepts of care management service delivery.
  • Knowledge and familiarity with public sector health coverage systems (e.g., Medicare / Medicaid or managed care plans).

Compensation

The pay range for this position is $23.00-25.00/hour.

Based on our compensation philosophy, an applicant’s placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
