ROLE OVERVIEW
The LifeStance Behavioral Care Coordinator (BCC) functions as a core member of our Integrated Behavioral Health (IBH) team. The BCC is responsible for supporting and coordinating patients' mental health care provided by the LifeStance IBH team of psychiatrists and therapists in consultation with a team of referring primary care physicians, and other mental health providers as appropriate. The BCC is also responsible for coordinating referrals to clinically indicated services as directed by the IBH team.
COMPENSATION: $21.00 - 23.00/hour
RESPONSIBILITIES
Support and closely coordinate mental health care with the patient’s care team, including the therapist, psychiatric consultant, primary care provider, and other treating mental health providers as indicated.
Communicate with clients by telephone regarding scheduling, referrals, insurance coverage, and document/communicate any changes and updates to IBH team through patient registry.
Facilitate LifeStance or other referrals as directed by the clinical team.
Perform patient benefit verification by coordinating with patient insurance plan representatives via phone and/or payor benefit portals.
Coordinate patient prior authorizations for specialty behavioral health services.
Participate in regularly scheduled caseload consultation with the IBH team psychiatrist and communicate any updates regarding the patients.
Facilitate patient engagement, communication, and follow-up in mental health care.
Match patients to clinicians based on need and utilize booking system to schedule necessary visits with LifeStance providers.
SKILLS & EXPERIENCE
High school diploma or equivalent.
Experience working with patients who have co-occurring mental health, and physical health problems.
Experience working with an EHR (Electronic Health Record); prior experience working with AdvancedMD would be advantageous.
Experience with screening and assessment for common mental / substance use disorders.
