What You'll Be Doing:

  • As a Care Manager on our Patient Operations team your work will support the patient population of a variety of programs and clients
  • You will work on the cutting edge of Telehealth, collaborating with clinical and operations leaders to assist the launch and everyday success of our programs

  • Immediate duties can include:
  • Identify high-risk and complex patients for program enrollment, communicating with all involved entities in the care of the patient.
  • The Care Manager utilizes research findings in practice and participates in transitional care programs
  • Supports Medication reconciliations/adherence programs; managing patient quality of life and functionality, both of acute and chronic disease states
  • Identifying and rectifying gaps in care, including assessment and support of patient ability for self careInitiates diagnostic and therapeutic measures as indicated by standing protocols.
  • Calls for crisis intervention if appropriate.
  • Communicates and acts as liaison with patients, family, and health professionals in a manner that respects confidentiality and dignity.
  • Charts concisely, descriptively, and legibly indicating patients progress or lack of progress.
  • Demonstrates compliance with existing standards and policies.
  • You will perform typical front office and back office responsibilities, including patient education, medication requests, handling patient questions, and supporting provider needs
  • Assist with patient support such as answering patient emails, patient phone calls
  • Work collaboratively with team members and our provider network to maintain an excellent model focused on patient care and high-quality service
  • Become an expert with our software solutions

What We're Looking For:

  • Education and Experience: Graduate of an accredited school of nursing, Current state license as a Licensed Practical Nurse/Registered nurse. Compact Licensure
  • Excellent communication skills
  • Critical thinking and problem solving
  • Time management and prioritization
  • Experience with a variety of Computer software and applications
  • Preferred: Experience with TelehealthPreferred: Experience with startups
