Acentra Health seeks a Clinical Team Lead to join our growing team.

Job Summary:

The CAP Clinical Team Lead (Social Worker) assists in overseeing the non-clinical team and functions, manages and completes assigned work plan projects to comply with performance standards. The CAP MSW Lead coordinates with the CAP/C and CAP/DA Program Managers and Clinical Director on decisions and issues and communicates with providers regarding determinations and other clinical matters. The CAP Lead, MSW also develops and leads non-clinical staff in activities to facilitate and support clinical, quality, and cost-effective outcomes. May require occasional travel within North Carolina.

Job Responsibilities:

    • With the CAP Program Manager's direction, oversee, coordinate, and monitor all non-Clinical team activities.
    • Leads team members in improving skills, creativity, and problem-solving.
    • Serve as the primary contact for CAP Human Service Workers(HSW) and Social Workers, MDT Review Team, and Service Plan Team.
    • Serve as primary contact and CAP orientation coordinator for new CAP hires (HSW/SWs).
    • Conduct ongoing training and education for staff to ensure competency in CAP Programs requirements, documentation standards, and HSW/SW protocols related to MDT Reviews, Service Plans, POCs, & Due Process.
    • Serve as a point of contact for inquiries related to the CAP Programs, providing guidance and support to staff and stakeholders.
    • Oversee and participate in completion of MDT Reviews and Service Plans, POC.
    • Oversee CAP Transitions: CAP/C to CAP/DA, Turning21, Innovations, & MFP.
    • With the CAP Program Manager's direction, monitors non-clinical staff productivity and performance to ensure alignment with performance standards outlined NCLIFTS (CIAE) contract.
    • Oversee completion of Monthly Service plan, POC quality checks.
